- Completed sports physicals are valid for 3 years.
- Sports physicals are different from "regular physicals" - Please be aware of this when making appointments.
- Providers in the area have sports physical forms of their own available, but the form is also available below.
- Medical doctors, as well as many chiropractors in town, can complete this service for you.
- You do not need to see your regular doctor for this!

Students that want to play a sport need a Sports Physical before they can participate. Chiropractors may be the easiest place to get an appointment. They already have the form, so just call and ask to set up an appointment for Sports Physical as soon as possible.
