- Completed sports physicals are valid for 3 years.
- Sports physicals are different from "regular physicals" - Please be aware of this when making appointments.
- Providers in the area have sports physical forms of their own available, but the form is also available below.
- Medical doctors, as well as many chiropractors in town, can complete this service for you.
- You do not need to see your regular doctor for this!

Students that want to play a sport need a Sports Physical before they can participate in practice or competitions. Chiropractors may be the easiest place to get an appointment. Providers already have the form - simply call and ask to set up an appointment for Sports Physical as soon as possible.
